    Your family must live in the State of Arizona to complete an Open Enrollment/Boundary Exemption Application.

    If your child is currently on an Open Enrollment/Boundary Exemption, please do not complete this application unless you are requesting a new school or you have recently changed Arizona addresses. 

    Please allow enough time to complete this application, as you will not be able to go back to complete.

  • EARLY ENTRANCE INTO KINDERGARTEN STATEMENT

    For your child to be considered for Early Entrance into Kindergarten, their 5th birthday must be between September 1-December 31, 2025. Your child will need an assessment to determine if they qualify for acceptance into kindergarten. Register online starting November 12, 2024 at www.CUSDcommunity.com to sign up for the Kindergarten Readiness Test. For additional questions, contact the Assessment office at 480-812-7674.

    School Information

  • SUSPENSION/EXPELLED STATEMENT

    If your child has ever been suspended or expelled, is currently suspended or expelled or in the process of being suspended or expelled, please ensure the Discipline Report is attached to this application. If you do not attach the Behavior/Discipline Report while filling out this application, you will have an opportunity to upload it using the Document Upload link that is provided in the email confirmation.

      • If your child does not have a discipline report, a statement on letterhead from the current school must be provided.

  • ANDERSEN K-5 STATEMENT

    Andersen Elementary doesn't have 6th grade students. Open Enrollment/Boundary Exemption Applications for 6th grade must be completed on the Secondary Open Enrollment Application, requesting Andersen Junior High. The Secondary Open Enrollment Application can be found by clicking here.

  • BOLOGNA DLI WRONG GRADE LEVEL STATEMENT

    The grade level you selected is not available for the Dual Lanaguage Immersion Spanish program at Bologna Elementary. Bologna only offers Kindergarten-5th grade Spanish Immersion. Please select a different school before moving to the next page.

  • CTA FREEDOM DLI WRONG GRADE LEVEL STATEMENT

    The grade level you selected is not available for the Dual Lanaguage Immersion Spanish program at CTA Freedom Campus. CTA Freedom only offers Kindergarten-3rd grade Spanish Immersion. Please select a different school before moving to the next page.

  • DUAL LANGUAGE IMMERSION STATEMENT

    In case your Dual Language Immersion (DLI) School Choice is at capacity (no seat available), you have the option to select the same school as a non-DLI School Choice. The application will remain on the capacity waitlist, with the time and date stamp of when it was received. If a seat becomes available with your requested DLI program, you will receive new approval email for the DLI program with your next steps for registration/transfer.

  • ELITE PERFORMANCE ACADEMY WRONG GRADE LEVEL STATEMENT

    The Elite Performance Academy does not offer Kindergarten-2nd grade instruction. Please select a different school.

  • SANTAN (K-6TH) STATEMENT

    Santan Elementary (Kindergarten-6th) and Santan Junior High (7th & 8th) are two different schools that have two different bell schedules, even though they share the same property.

    CUSD is fortunate to have three different models to meet the needs of students who are identified as gifted. We offer consultative services at all elementary schools, self-contained satellite classrooms and a school-wide self-contained program. You can view a description of these models by clicking here.

    Your Category 2 request cannot be reviewed for Gifted Education until Gifted Test scores have been submitted.

  • GIFTED EDUCATION SCHOOL CHOICE REVIEW CRITERIA

    Your Category 2 request for a gifted program transfer will be considered by the date and timestamp of the Open Enrollment/Boundary Exemption Application, if Student Services - Gifted Education has your child's Gifted Test Scores on file or you have attached them to this application from another school district or private tester.

    If your child takes a Gifted Test after the submission of this Open Enrollment Application/Boundary Exemption, the date of the Gifted Test Scores will supersede the Open Enrollment Application date.

    Your application will be reviewed by the Student Personalized Learning (Special Education) office in the order that it was received if the Student Peronalized Learning office has your student's IEP on file because your child currently attends a CUSD school or you have attached the full IEP to this application because your child currently does not attend a CUSD school.

    1. I acknowledge that transportation of the student is the responsibility of the parent/legal guardian. CUSD bus transportation is NOT provided.

    2. I acknowledge that record of excessive absences or truancies in this District or any other public or private school may result in the application being denied or admission being revoked.
